NEMT Credentialing in Florida

What makes Florida different is that credentialing is not one application - it is two layers, and the second layer repeats per broker and per region. First you enroll as a Medicaid transportation provider through the Florida Medicaid Provider Portal (FMMIS enrollment). Then, because Statewide Medicaid Managed Care 3.0 - launched February 1, 2025 - carves NEMT into managed care, you credential separately with the transportation broker each managed care plan uses in each of the nine regions you serve. Roughly eight plans operate across those nine regions, and each picks its own broker: Modivcare for plans like Humana, Simply, Aetna and Community Care Plan, MTM for UnitedHealthcare, Alivi for Sunshine Health, and Ride2MD for Florida Community Care. Every driver who takes a Medicaid trip needs a complete, current file: a valid Florida license for the vehicle class they drive, a clean motor-vehicle record, a criminal background check, drug and alcohol screening, and proof of the required training each broker asks for. The catch in Florida is that you are credentialing the same drivers into several broker networks - Modivcare, MTM, Alivi, Ride2MD - and each has its own file format and its own renewal clock.
