What are California's NEMT vehicle and VSSI requirements?

Since March 27, 2025, every Medi-Cal NEMT vehicle needs a Vehicle Safety Systems Inspection (VSSI) certificate from the California Bureau of Automotive Repair — and the older brake-and-light certificate is no longer accepted. A van without a current VSSI certificate cannot be credentialed to run NEMT trips, full stop. On top of VSSI, each vehicle still needs current California registration and title, the vehicle listed on your auto liability policy, and — for wheelchair and gurney vehicles — proof the securement equipment and ADA accessibility meet the standard. What does California NEMT driver credentialing require?

Every driver who takes a Medi-Cal trip needs a complete, current file: a valid California license for the vehicle class they drive, a clean motor-vehicle record, a criminal background check, drug and alcohol screening, and proof of the required training — typically defensive driving, CPR and First Aid, and wheelchair securement or a passenger-assistance course. Driver credentialing is where an operating fleet gets caught later: a license or a background check quietly expires and the driver can no longer run trips.
